Dameron Hospital $650,000 Data Breach Settlement

If you received a data breach notification from Dameron Hospital Association indicating that your personal information was compromised, you may be eligible to claim a payment from a class action settlement.

Dameron Hospital Association has agreed to pay $650,000 to settle a class action lawsuit due to a cyberattack that allegedly exposed personal and protected health information of approximately 262,475 class members.

Who is eligible for a settlement payout?

• Settlement Class Members: Individuals in the United States notified by Dameron Hospital that their personal and patient information was compromised in the cybersecurity incident.
• California Subclass Members: California residents who received notice of the data incident at their California address.

What type of payout can I submit for?

• Credit Monitoring Services: Class members that do not select the Alternate Cash Payment can claim 12 months of three-bureau credit monitoring services.
• Compensation for Unreimbursed Economic Losses: Class members that do not select the Alternate Cash payment can also submit a claim for up to $5,000 for documented financial losses traced to the data incident.
• Alternative Cash Payment: Class members that do not submit for Credit Monitoring or Economic Losses may submit a claim for a cash payment of $50. California Subclass Members qualify for a $100 alternative payment.

The amounts for the alternative cash payment will be adjusted based upon the number of approved claims.

Do I need to include documentation with my claim?

The following is required:

• Class Member ID required to submit online
• Unreimbursed Economic Losses claims require documented proof of financial losses such as receipts, bank/credit card statements, credit repair fees, fees for professional services, and more.

How to claim compensation

To claim a settlement payment, you must must complete and submit a claim form by the April 22, 2025 deadline. You can file a claim online or download the PDF claim form to print and mail to the settlement administrator.

• Electronic payment, such as PayPal or Venmo

The settlement fund will cover expenses and payouts:

• Credit monitoring services: TBD
• Payments to claimants: based on valid claims submitted
• Settlement administration costs: TBD
• Attorneys' fees: up to $216,645 plus expenses
• Service awards: $2,500 for the named plaintiff

When is the Dameron Hospital Settlement payout date?
