To be considered a class member eligible to file a claim, you must meet the following criteria:

• You reside in the United States and you are a current or former Hair Club for Men employee.
• You received a Notice Letter from Hair Club for Men Ltd., Inc. informing you that your personal information may have been involved in the October 2023 security incident.

How much is the class action settlement payment?

Claimants will select one of two benefit options:

• Benefit Option 1: You can submit for documented Unreimbursed Losses of up to $4,500, plus three years of credit monitoring services.
• Unreimbursed Losses include service fees, costs associated with freezing or unfreezing credit, and monetary losses due to fraud or identity theft traceable to the data breach.

• Benefit Option 2: Class members that do not claim Benefit Option 1 can submit for an Alternative Cash Payment of $75.

To receive a settlement payment, class members must submit a claim by July 17, 2025:

• You can print the PDF claim form and mail it to the Settlement Administrator.
