On March 14, 2025, CDHA Management, LLC and Spark DSO, LLC, doing business as Chord Specialty Dental Partners, publicly disclosed a data breach involving unauthorized access to several employee email accounts. The breach was initially discovered on or around September 11, 2024, after the company noticed suspicious activity related to an employee's email account.

Upon discovering the incident, Chord Specialty Dental Partners immediately secured the compromised account and began an extensive investigation with the assistance of third-party cybersecurity specialists. This investigation revealed that an unauthorized individual gained access to multiple email accounts over a period from August 19, 2024, to September 25, 2024.

As a result of this unauthorized access, the personal information of approximately 173,430 individuals was potentially exposed. The types of personally identifiable information (PII) and protected health information (PHI) involved include names, addresses, Social Security numbers, driver's license numbers, bank account details, payment card information, dates of birth, medical information, and health insurance information.

Currently, Chord Specialty Dental Partners has not found evidence indicating misuse of the exposed information; however, the possibility of unauthorized access to the data cannot be ruled out.

Chord Specialty Dental Partners reported the breach to the U.S. Department of Health and Human Services on March 14, 2025. You can view the official breach disclosure on the Department's breach reporting portal.

You can request your free credit reports by visiting AnnualCreditReport.com or by calling 1-877-322-8228.

Additionally, you have the right to place a fraud alert or credit freeze on your credit file at no cost. A fraud alert warns creditors to verify your identity before issuing credit, while a credit freeze restricts access to your credit report entirely, preventing new credit accounts from being opened without your consent.

To place either a fraud alert or credit freeze, contact the following credit bureaus directly:
