
If you received a notice from Sur La Table about a data incident in May 2023, you may be eligible to claim up to $4,000—or more in some cases—from a class action settlement.
SLT Lending SPV, Inc., which is the holding entity for Sur La Table, has agreed to pay $550,000 to settle a class action lawsuit related to a data breach that occurred between March 15, 2023, and March 25, 2023.
The lawsuit alleges that Sur La Table failed to implement reasonable security measures, which resulted in unauthorized access to sensitive personal information. This information included names, driver’s license or state identification numbers, medical or health information, and for some individuals, Social Security numbers, dates of birth, and health insurance plan enrollment details.
Who can file a claim?
To be eligible for compensation, you must have:
- Received a notice from Sur La Table (SLT Lending SPV, Inc.) about the data incident on or around May 24, 2023
- and your name appears on the official list of individuals to whom the company sent notice regarding the incident
- and you have a Unique ID included in your settlement notice, which is required to file a claim

There are two groups within the class:
- General Class Members: Anyone who received notice of the data incident.
- California Subclass Members: Individuals who received notice and were California residents at the time of the incident. California Subclass Members may be eligible for additional compensation.
How much can class members get paid?
The settlement provides several types of awards, depending on your circumstances and location:
- Reimbursement for Out-of-Pocket Losses: Up to $4,000 per class member for documented, unreimbursed expenses that are "fairly traceable" to the data incident. You may submit multiple claims for out-of-pocket losses, but the total reimbursement cannot exceed $4,000 per class member.Eligible expenses include:
- Costs associated with fraud or identity theft (such as professional fees or credit repair services)
- Miscellaneous expenses (notary, fax, postage, copying, mileage, long-distance telephone charges)
- Costs for credit monitoring or other mitigative services incurred on or after March 15, 2023
- Alternative Cash Payment: If you did not incur out-of-pocket losses, you may claim a pro rata cash payment. The total remaining settlement fund (after all other claims and expenses are paid) will be divided equally among those who submit a valid claim for this benefit.
- California Statutory Payment: California Subclass Members can claim up to $100 as a statutory payment under the California Consumer Privacy Act (CCPA). This is in addition to any other benefits under the settlement.
- Identity Theft Protection and Credit Monitoring: All class members who submit a claim are eligible for two years of free credit monitoring and identity theft protection through IDX. This service includes monitoring of at least one major credit bureau, identity restoration and recovery services, and $1,000,000 in identity theft insurance.
How to submit a claim
To claim an award, you must submit a claim form. You can file a claim online or download the PDF claim form and mail it to the settlement administrator.

If you are a California resident, you can check the box for the $100 statutory payment. All class members can opt in for two years of credit monitoring by checking the appropriate box on the claim form.
To claim reimbursement for out-of-pocket losses, you must provide:
- Your name and current address
- Documentation supporting your claim (such as receipts, bills, or other non-self-prepared documents)
- A brief description of the loss, if not clear from the documentation
- A verification statement that your claim is true and correct, under penalty of perjury
For the alternative cash payment or California statutory payment, you must provide your Unique ID and complete the claim form, but no additional documentation is required unless you are claiming out-of-pocket losses.
Claim instructions
- Gather your Unique ID from the notice you received.
- Decide which benefit(s) you wish to claim: out-of-pocket losses, alternative cash payment, California statutory payment, and/or credit monitoring.
- If claiming out-of-pocket losses, collect all supporting documentation (receipts, bills, etc.).
- Go to the online claim form or download and print the PDF claim form.
- Complete all required fields, including your name, address, Unique ID, and details about your claim.

- If filing online, upload your documentation as instructed.
- Submit your claim by July 7, 2025.
Payout options
- If you file by mail, you will receive payment by check.
- If you file online, you can choose to receive payment electronically.
- All checks will expire and become void 180 days after being issued.
$550,000 settlement fund
The settlement fund of $550K will cover all payments to class members, attorneys’ fees and costs, service awards to class representatives, and administrative expenses.
- Attorneys' fees: Up to $183,333.33 (one-third of the fund)
- Attorneys' expenses: Up to $15,000
- Service awards to class representatives: Up to $2,000 each
- Settlement administration costs: TBD
- Payments to class members: Remainder of the fund

When is the Sur La Table Data Incident Settlement payout date?
Payments will be issued after the court grants final approval and any appeals are resolved. The final approval hearing is scheduled for August 27, 2025. Payments may take several months after the hearing to be processed and distributed.
