If you received a notification from Hi-School Pharmacy that your personal information may have been compromised in a November 2023 data incident, you may be eligible to submit a claim for up to $5,000 from a class action settlement.

Hi-School Pharmacy Services LLC and Hi-School Pharmacy, Inc. have agreed to pay $600,000 to settle a class action lawsuit. The lawsuit was filed after a cyberattack in November 2023, during which files containing sensitive personal information of approximately 17,662 individuals were potentially accessed.

Who is eligible for a data breach payment?

Class members must meet the following criteria:

  • You are a U.S. resident and your personal information was compromised in the November 2023 data incident involving Hi-School Pharmacy.
  • You received a notification from Hi-School Pharmacy about the data breach.

How much is the class action settlement payout?

Claimants have the following options:

Documented Out-of-Pocket Losses: Class members can submit for up to $5,000 per person for unreimbursed monetary losses traceable to the data incident. Expenses and losses include:

  • Losses related to fraud or identity theft
  • Professional fees, such as attorneys’ fees, accountants’ fees, and fees for credit repair services
  • Costs associated with freezing or unfreezing credit with any credit reporting agency
  • Credit monitoring costs incurred after the incident and before submitting your claim
  • Credit Monitoring: You can also claim two years of free three-bureau credit monitoring and identity theft protection.
  • Alternative Rata Cash Payment: Class members that do not submit for Out-of-Pocket Losses or Credit Monitoring can submit a claim for a pro rata cash payment. The final amount will be determined by the number of valid claims filed.

How to claim a data breach settlement payment

To receive a class action rebate, class members must submit a claim form. You can file a claim online using the online claim form. Or the PDF claim form can be printed and mailed to the settlement administrator.

All claims must be submitted online or postmarked by the July 9, 2025 deadline.

What proof or information is required with a data breach claim?

  • The Unique ID and PIN from official notice is required to submit an online claim.
  • Document Out-of-Pocket Losses claims require supporting documentation such as receipts, invoices, bank or credit card statements, police report or other documentation showing unreimbursed expenses and financial losses due to fraud caused by the data incident.

$600K data breach settlement fund breakdown

The $600,000 settlement fund will cover all costs associated with the settlement:

  • Settlement administration costs: To be determined
  • Attorneys' fees and expenses: Up to $198,000
  • Service award to class representative: $3,000
  • Credit monitoring services: Cost determined by number of claims submitted
  • Payments to class members with approved claims: Remainder of settlement funds

When is the Hi-School Pharmacy data breach settlement payment date?

The final hearing will take place on July 31, 2025. Payments will be issued to approved claimants after the court grants final approval and all claims are processed.
