  • Documented Losses (up to $5,000): You must provide reasonable documentation for expenses such as unreimbursed bank fees, long-distance phone charges, cell phone charges (if charged by the minute), data charges (if charged by usage), postage, gasoline for local travel, and fees for credit reports, credit monitoring, or identity theft insurance purchased between January 30, 2023, and the claim deadline. Acceptable documentation includes telephone records, receipts, and correspondence. Personal certifications or affidavits are not considered sufficient documentation but may be submitted for clarification.
  • Flat Cash Payment ($75): If you do not wish to provide documentation, you may claim a flat cash payment of $75 by simply submitting the claim form.
  • Credit Monitoring: All class members can claim up to three years of free credit monitoring, regardless of whether they claim a cash payment.
