• Credit Monitoring and Identity Theft Protection Services: Up to three years of services.
• Reimbursement for Out-of-Pocket Losses: Up to $7,000 for documented expenses.
• Compensation for Lost Time: Up to five hours at $25 per hour.
• Alternative Cash Payment: A pro rata share of the remaining settlement fund.

The exact amount of the alternative cash payment will depend on the number of valid claims submitted and the remaining funds after other expenses are covered. For example, if $500,000 remains after expenses and 10,000 valid claims are submitted, each claimant would receive $50.

How to Claim an Award

To claim an award, class members must submit a claim form. You can file a claim online or download the PDF claim form and mail it to the Settlement Administrator.

For out-of-pocket losses, documentation such as receipts or account statements must be provided. However, no documentation is needed for claiming lost time or credit monitoring services.
