If you received notice from Evergreen Treatment Services that your private information was potentially impacted by a data incident in December 2022, you may be eligible to claim $100 and up to $10,000 from a class action settlement.

Evergreen Treatment Services has agreed to pay $425,000 to settle a class action lawsuit for allegedly exposing the private information of its current and former patients due to a data breach.

Who can file a claim?

You are eligible to be part of this settlement if:

• You received a notice from Evergreen informing you of the data incident.

How much can class members get?

• Option 1: Three years of credit monitoring services, compensation for unreimbursed economic losses up to $10,000, and compensation for lost time up to 6 hours at $25 per hour, totaling $150.
• Option 2: An estimated cash payment of $100.

The actual cash payment may vary based on the number of valid claims filed. If the settlement fund is insufficient to cover all claims, payments will be adjusted on a pro rata basis.

For example, if there are more claims than the fund can cover, each claimant might receive less than $100.

When is the Evergreen Treatment Data Breach payout date?
