If you received a prerecorded voice message from Choice Health Insurance, you may be eligible to claim $30 from a class action settlement.
Choice Health Insurance has agreed to pay $994,799.28 to settle a class action lawsuit for alleged violations of the Telephone Consumer Protection Act (TCPA).
Who can file a claim?
The settlement class includes individuals who received a prerecorded voice message from Choice Health Insurance. Specifically, the class members are:
- Individuals in the United States who received a prerecorded voice call from Choice Health Insurance, LLC between July 28, 2018, and September 18, 2024
- The calls must have been made regarding Choice Health Insurance's property, goods, and/or services
- The lead data for these calls must have been provided by ZeetoGroup, LLC to Digital Media Services, LLC
If you meet these criteria, you are considered a class member and may be eligible for a settlement award.
How much can class members get?
Class members who submit a valid claim form are eligible to receive a payment of $30. This amount is subject to deductions for notice and administration costs, attorneys' fees, and any service award to the plaintiff.
The settlement fund of $994,799.28 will be used to cover these payments. If the number of claims exceeds the available funds, the payments may be adjusted on a pro-rata basis. For example, if the total claims exceed the settlement fund, each claimant's award will be reduced proportionally.
How to claim an award
To claim an award, class members must submit a claim form. You can file a claim online or download a PDF claim form to mail. The completed form must be postmarked by December 26, 2024.

When is the Choice Health Insurance payout date?
The court will hold a final approval hearing on December 11, 2024, to decide whether to approve the settlement. If approved, payments will be distributed to claimants who submitted valid claims within 60 days following the final approval of the settlement and resolution of any appeals.
