Who can file a claim?

You are part of the settlement class if:

• You received a notification from Bridgeway Center in May 2024 about the potential impact on your personal information.

How much can class members get?

• Ordinary Losses: Up to $1,500 per person for documented expenses like bank fees, credit monitoring, and other out-of-pocket costs incurred due to the incident.
• Extraordinary Losses: Up to $5,000 per person for documented, unreimbursed monetary losses due to fraud or identity theft directly linked to the incident.
• Flat Payment: Alternatively, a flat payment of $125 is available.

Your cash payment may be subject to a pro rata adjustment depending on the number of valid claims. For example, if the total valid claims exceed the net settlement fund, the amount each claimant receives might be reduced proportionally.

Is proof required to submit a claim?

Claimants need to provide documentation to support their claims. Documentation requirements include:
