On February 23, 2025, Brainard Surgery Center detected suspicious activity on its computer network, prompting an immediate investigation. With the help of third-party cybersecurity and digital forensics specialists, the center determined that certain files were taken from its systems by an unauthorized actor. The exact number of individuals affected has not yet been disclosed, as a comprehensive review of the impacted files is still ongoing. However, the breach is significant in scope, given the types of information involved and the nature of the facility.
The exposed information varies by individual but may include: name, address, date of birth, Social Security number, driver’s license number or other state identification number, medical claims information, clinical information (such as diagnosis/conditions, medications, and other health information), and health insurance information.
This means both personally identifiable information (PII) and protected health information (PHI) were at risk.
The center has not yet disclosed who was responsible for the attack, and the review to determine the full scope of the breach is still underway. Once the review is complete, affected individuals will receive notification letters via U.S. mail. For now, detailed information about the breach and recommended steps for affected individuals can be found in the official notice posted by Brainard Surgery Center.
In response to the incident, Brainard Surgery Center LLC immediately launched an investigation and engaged cybersecurity and digital forensics experts to assess the nature and scope of the breach. The center has implemented additional security measures to help prevent similar incidents in the future. While the investigation and file review are ongoing, Brainard Surgery Center LLC is preparing to notify affected individuals directly by mail once their information is confirmed to have been involved.
The center has also established a dedicated call center to assist those who may have questions or concerns about the breach. Individuals can call 1-888-830-8199, Monday through Friday from 9:00 am to 5:00 pm ET, or write to Brainard at 29017 Cedar Road, Lyndhurst, OH 44124.
Given the sensitive nature of the exposed information—including Social Security numbers and medical data—affected individuals are strongly encouraged to remain vigilant for signs of identity theft or fraud. It is advisable to review account statements, explanation of benefits, and credit reports for suspicious activity. Resources such as AnnualCreditReport.com, Equifax, Experian, and TransUnion are available for obtaining free credit reports and placing fraud alerts or credit freezes. Additional guidance on protecting your information is included in the official notice.
