On April 14, 2025, Alternate Solutions Health Network, LLC (ASHN), a provider of home health and hospice care, disclosed a significant data breach affecting 93,589 individuals in the United States.

The incident was reported to the U.S. Department of Health and Human Services and involved unauthorized access to a company email account. The breach was first detected when ASHN discovered that, on or around May 30, 2024, an unauthorized actor had accessed one of its email accounts.

After a thorough investigation with external cybersecurity professionals, ASHN determined on February 14, 2025, that the compromised account contained sensitive personal and health information.

The exposed information varied by individual and included personally identifiable information (PII) such as first and last names, dates of birth, addresses, driver's license numbers, and Social Security numbers.

In addition, protected health information (PHI) was exposed, including physician and clinician names, clinical information, diagnostic details, and treatment information. The breach was the result of unauthorized access and acquisition of data from the affected email account, making this a serious incident due to the sensitive nature of both PII and PHI involved.

Alternate Solutions Health Network's response

Upon learning of the breach, ASHN immediately secured the compromised email account and began a comprehensive investigation with the help of external cybersecurity experts. The company has since implemented additional cybersecurity safeguards, enhanced employee cybersecurity training, and improved its cybersecurity policies and protocols to help prevent similar incidents in the future.

ASHN began mailing notification letters to affected individuals on April 14, 2025, to the extent valid mailing addresses were available. If you did not receive a letter but believe you may be affected, you can call 1-877-724-0180 for more information. The company has provided a dedicated response line staffed by professionals familiar with the incident and knowledgeable about steps you can take to protect your information. The response line is available Monday through Friday, 9:00 AM to 9:00 PM Eastern time.

Given the nature of the breach—unauthorized access to an email account containing both PII and PHI—it is important for affected individuals to remain vigilant against identity theft and fraud.
