You are a United States resident and a current or former patient who received a notice letter from Mena Regional Health System notifying you that your personal information was compromised in the data breach incident that occurred around November 8, 2022.
How much will my class action settlement payment be?
Class members can file for one or both of these options:
- Pro Rata Cash Payment: After deducting out-of-pocket expense claims and all other settlement costs, the remaining settlement fund will be distributed proportionally. Amount will be determined by the number of claims filed.
- Out-of-Pocket Expense Reimbursement: Class members can submit for reimbursement of documented monetary losses directly related to the data breach, up to a maximum of $2,000 per individual. Losses include unreimbursed charges related to fraud or identity theft, professional fees paid to attorney, accountant, or credit repair services, and other expenses.
- Expenses must have been incurred on or after September 1, 2023, through the date you submit your claim.

To receive a payment, class members must submit a valid claim form by the deadline of June 12, 2025. You can file a claim online or download and print a PDF claim form to mail to the settlement administrator.
