If you were notified that your private information may have been compromised in the Practice Resources, LLC Data Breach, you may be eligible to claim compensation from a class action settlement.

Practice Resources, LLC has agreed to pay $1,500,000 to settle a class action lawsuit for alleged negligence in protecting consumer data, leading to a cybersecurity breach.

Who can file a claim?

You are part of this settlement as a Settlement Class Member if your private information, including full names, home addresses, dates of treatment, health plan numbers, and/or medical record numbers, may have been compromised in the data breach.

This includes all individuals who were sent the Notice of Data Privacy Incident on or around August 23, 2022.

How much can class members get?

The Net Settlement Fund will first cover valid claims for credit monitoring and insurance services. If funds remain, they will be used for documented losses and then divided among cash payment claims. If the claims exceed the fund, payments will be prorated.

  • Compensation for Documented Losses: Up to $5,000 for each Settlement Class Member who submits a valid claim form and opts for reimbursement. This requires documentation such as credit card statements, bank statements, invoices, and receipts.
  • Credit Monitoring and Insurance Services: Up to three years of services provided by CyEx, LLC, including three-bureau credit monitoring and $1,000,000 in identity theft insurance.
  • Cash Payment Option: A pro rata payment from the Net Settlement Fund, subject to the number of valid claims and available funds.

If the total valid claims for credit monitoring and insurance services exceed the total amount of the Net Settlement Fund, the duration of the coverage will be reduced to exhaust the fund. If that happens, no money will be distributed to valid claims for documented losses or for cash payments.

Proof required to submit a claim

Claimants must provide documentation for documented losses, such as:

  • Credit card statements.
  • Bank statements.
  • Invoices and receipts.

For credit monitoring and insurance services, no additional documentation is needed beyond the claim form.

How to claim an award

To claim an award, complete and submit a Claim Form by January 27, 2025. You can file a claim online or download the claim form from the settlement website when they become available. Completed forms can be mailed to:

When is the Practice Resources Data Breach payout date?

After the final approval hearing on February 12, 2025, if the settlement is approved, the claims administrator will begin processing payments. Ensure your claim is submitted by the deadline to be eligible for compensation.
