If you were notified that your private information may have been compromised in the Practice Resources, LLC Data Breach, you may be eligible to submit a claim for payment from a class action settlement.

Practice Resources, LLC has agreed to pay $1,500,000 to settle a class action lawsuit for alleged negligence in protecting consumer data, leading to a 2022 cybersecurity attack.

Who can submit for a settlement payout?

Class members include the following:

  • The 924,138 patients that may have had their private information compromised in the cybersecurity incident
  • All individuals who received a Notice of Data Privacy Incident on or around August 23, 2022

How much are payouts per claimant?

Class members may submit a claim for ONE of the following

  • Credit Monitoring and Identity Theft Insurance: Up to three years of services provided by CyEx, LLC, which includes three-bureau credit monitoring and $1,000,000 in identity theft insurance.
    • Claimants who chose a previous credit monitoring offer or procured services on your own, can delay the activation of the CyEx, LLC services for up to 12 months
  • Compensation for Documented Losses: Up to $5,000 for documented financial losses due to the incident.
  • Cash Payment Option: A pro rata payment from remaining settlement funds after Credit Monitoring Services and Document Loss claims are paid

Claims will be paid in the following order: 1. Credit Monitoring and Identity Theft Services, 2. Documented Losses, 3. Cash Payment Option. If the number of Credit Monitoring claims exceeds the net settlement fund amount, then Document Loss and Cash Payment Option claims will not be paid.

How do I claim my settlement payment?

To claim credit monitoring services or a cash payout, complete and submit a Claim Form by April 25, 2025. You can file a claim online or download the PDF claim form from the settlement website to print and mail

Is supporting documentation required?

Yes, some proof and documentation is necessary:

  • Claim ID and PIN from notice is required to submit an online claim form
  • Documentation must be provided with a Documented Losses claim such as credit card or bank statements, invoices and receipts
  • Additional documentation is not required to submit a claim for Credit Monitoring or the Cash Payment Option

$1,500,000 Settlement fund

The settlement fund of $1,500,000 will cover:

  • Credit monitoring/Identity theft services, Documented Losses and Cash Payment Option claims
  • Settlement notice and administrative expenses
  • Attorneys' fees of up to $500,000 plus litigation expenses
  • Service award payments for Class Representatives of up to $2,500 each

When is the Practice Resources Data Breach payout date?
