Employees and former employees of United of Omaha Life Insurance who have been notified that their private information may have been accessed in a July 2024 cybersecurity incident.
How much will the data breach settlement payment be?
Class members can submit for credit monitoring and one of the payment options below:
- Reimbursement for Out-of-Pocket Losses: Up to $1,500 for monetary losses traceable to the data breach, including up to four hours of lost time at $20 per hour.
- Alternative Cash Payment: $50 if an Out-of-Pocket Losses claim is not filed.
- Credit Monitoring & Identity-Theft Protection: Two years of credit monitoring and identity restoration services.
How to claim a class action payout
To claim an award, class members must complete and submit a claim form by the May 29, 2025 deadline. An online claim can be submitted or a PDF claim form can be printed and mailed to the settlement administrator.

When is the Omaha Life Insurance data incident payout date?
The court will hold a final approval hearing on June 25, 2025. Claims will be process and payments issued to approved class members after the judge grants approval.
