If you received a notice from HealthAlliance, Inc. indicating that your personal information may have been compromised in a data incident, you might be eligible to claim a financial award from a class action settlement.

HealthAlliance, Inc. has agreed to pay $1,290,000 to settle a class action lawsuit regarding alleged unauthorized access to personal information, including sensitive health and financial data, between August 18, 2023, and October 13, 2023.

Who can file a claim?

To be eligible, you must meet the following criteria:

  • You must have received a notification from HealthAlliance, Inc. regarding the data incident.

  • The data incident must have potentially impacted your personal information, which includes names, addresses, Social Security numbers, and health-related records.

How much can class members get?

Class members can choose between two cash payment options or opt for credit/medical monitoring services.

  • Cash Payment A: This option includes compensation for ordinary losses, lost time, and extraordinary losses. Ordinary losses may be reimbursed up to $7,500 per person for expenses such as bank fees, phone charges, and credit monitoring fees incurred due to the data incident. Lost time can be compensated at $25 per hour for up to four hours. Extraordinary losses require documentation of actual monetary loss due to fraud or identity theft.
  • Cash Payment B: A flat payment of $100 is available without the need for documentation.

In addition to cash payments, class members can claim up to three years of credit/medical monitoring services valued at $179.40 per year.

Pro Rata Adjustments

If the total valid claims exceed the net settlement fund, cash payments may be reduced on a pro rata basis. Conversely, if the claims do not exhaust the fund, payments may increase proportionally. The settlement administrator will prioritize credit/medical monitoring payments before cash payments.

Is proof required to submit a claim?

Yes, claimants must provide documentation to claim compensation for ordinary and extraordinary losses under Cash Payment A. This includes receipts or other non-self-prepared documentation demonstrating the costs incurred.
