Who is eligible for a settlement payment?

To be part of the settlement class, you must meet the following criteria:

  • You received a notice from Enzo informing you that your personal information may have been compromised.

How much is the data breach payout?

Class members have the following options:

  • Documented Loss Payment: Up to $10,000 for documented financial losses related to the network incident.
  • Cash Fund Payment: Class members that do not submit a Documented Loss claim are eligible for a pro rata cash payment. Amount will be determined by the number of claims filed.
  • Healthcare Data and Credit Monitoring Services: Two years of coverage through Medical Shield Premium.

How to claim an Enzo settlement payment

To claim a payout, class members must complete and submit a claim form by the June 23, 2025 deadline. This can be done by submitting online or by printing the PDF claim form and mailing it to the settlement administrator.

Required proof

  • Notice ID and Confirmation Code required to submit online.

$7.5 Million data breach settlement fund

The settlement fund of $7,500,000 million will include various costs and payments:

  • Attorneys' fees and expenses: Up to $2,475,000
  • Service awards to class representatives: $1,500 each for 16 representatives
  • Medical Shield Premium services: Determined by number of valid claims
  • Payments to approved claimants: Remaining funds after other expenses
