Class members have the following claim options:

  • Identity Theft Protection Services: All claimants can submit for one year of identity theft protection services, including up to $1 million in identity theft insurance.
  • Reimbursement for Out-of-Pocket Expenses: Reimbursement for documented out-of-pocket expenses traceable to the CCH data breach. Claims can be submitted for up to $350 for employees and $250 for patients.
  • Lost Time Reimbursement: Time spent dealing with the incident can claim up to 4 hours at $25 per hour.
    • If this benefit is claimed in addition to Out-of-Pocket Expenses, they will be combined and subject to the monetary cap.
  • Reimbursement for Actual Monetary Loss: If you suffered documented financial losses due to the cybersecurity incident, you can claim up to $3,500 for employees and $1,500 for patients.

Is supporting documentation required to receive a payout?

Documentation and proof required:

  • Unique ID and PIN from notice required to submit an online claim for any of the options
  • Out-of-Pocket Expenses claims require proof of documented expenses directly connected with the breach, such as bank statements, receipts, and proof of fees paid.
  • Reimbursement of Actual Monetary Loss claims require proof of documented financial losses that you were unable to get reimbursed from other sources.

When will claims be paid out?
