  • Reimbursement for Lost Time: Payment for up to 4 hours at $21.25 per hour for time spent addressing issues related to the breach.
  • Alternative Cash Payment: A flat payment of $65 for class members that do not claim reimbursement for Ordinary Losses, Extraordinary Losses, or Lost Time.
  • Identity Theft Protection and Credit Monitoring: Two years of services at no cost, which includes monitoring with all three credit bureaus and $1,000,000 in identity theft insurance.

How to claim a data breach settlement payment

To claim an award, you must complete and submit a claim form by the May 14, 2025 deadline. You can file a claim online or print the PDF claim form and mail it to the settlement administrator.
