• Alternative Cash Payment: A one-time payment of $25 if no other benefits are claimed.

Claim documentation

Claimants must provide documentation for ordinary and extraordinary losses. For lost time, a narrative statement under penalty of perjury is needed if no documentation is available.

• Ordinary Losses: Documentation of actual, unreimbursed monetary loss.
• Extraordinary Losses: Documentation of significant, unreimbursed monetary loss caused by the data incident.
