- Lost Time: Compensation of $25 per hour for up to 4 hours, totaling up to $100, for time spent dealing with the data incident.
- Extraordinary Losses: Up to $5,000 for significant documented losses that are not covered by other categories.
- Alternative Cash Payment: A one-time payment of $25 if no other benefits are claimed.
Claim documentation
Claimants must provide documentation for ordinary and extraordinary losses. For lost time, a narrative statement under penalty of perjury is needed if no documentation is available.
