If you had private health information exposed in the Cyberattack involving Advanced Medical Management, LLC (AMM) and were sent notification of the Cyberattack from AMM or were subject to the notice published in the media and on AMM’s website, you may be eligible to claim up to $7,500 from a class action settlement.
Advanced Medical Management has agreed to pay $2,500,000 to settle a class action lawsuit for alleged failure to adequately protect personal and health information, resulting in unauthorized access by a third-party criminal actor.
Who Can Claim an Award?
To be part of the 319,485 person Settlement Class, you must meet specific criteria.
You are included if you reside in the United States and had private health information exposed in the cyberattack involving AMM.
If you received a notice by mail, you are likely a Settlement Class Member. If you did not receive a notice by mail but believe you are part of the Settlement Class, you can contact the Settlement Administrator for verification.
What Awards Can Class Members Claim?
Class Members can claim two types of awards: Documented Loss Payments and Cash Fund Payments.
Documented Loss Payments
Documented Loss Payments reimburse up to $7,500 for out-of-pocket losses that are fairly traceable to the cyberattack. They cover:
- Unreimbursed costs, expenses, amounts, or charges incurred due to identity theft or fraud.
- Costs associated with accessing or freezing/unfreezing credit reports.
- Miscellaneous expenses such as notary, fax, postage, copying, mileage, and long-distance telephone charges.
- Other mitigative costs incurred from May 10, 2023, through the date of the claim submission.
- Unpaid time off work to address issues fairly traceable to the cyberattack.
Cash Fund Payments
Cash Fund Payments are calculated pro rata, meaning the amount may increase or decrease depending on the number of claims. For example, if fewer claims are submitted, each claimant may receive more than the estimated $50.
Is Proof Required to Claim the Reward?
Yes, proof is required to claim documented loss payments. Claimants must provide documentation, such as credit card statements, bank statements, invoices, telephone records, and receipts, to support their claims.
No additional documentation is needed for cash fund payments if the claimant provides their Unique ID Number from the mailed notice.
