The investigation revealed unauthorized access and copying of certain information. By December 5, 2024, CODAC identified the specific sensitive and personal information affected.
The types of consumer information exposed include:
- Name
- Date of birth
- Medical diagnosis and treatment information
- Health insurance information
- Medical record number
- Date of service
- Social Security number
The recent data breach disclosures confirm August claims by the ransomware group, Qilin, that CODAC had fallen victim to a ransomware attack.
